Family Medicine Academic centers at the front serving LGBTQ communities

Summary

LGBTQ+ rights have made significant progress over the past few years. Unfortunately, this progress has been inconsistent. Globally, LGBTQ+ people still face widespread stigmatization and persecution, either through informal mechanisms or legal discrimination. In a surprising number of countries, the penalty for being in a same–sex relationship is prison or even death.
As family medicine physicians, it is our duty to be aware of the social disparities the LGBTQI+ community continues to face. Access to and quality of care is frequently limited by legal restrictions, social stressors, limited family support, and lack of community acceptance.
Multiple institutions across the USA have incorporated sexual and gender minority health in medical school curricula at the undergraduate and graduate levels. However, there are currently no universal curricular guidelines in the US related to LGBTQ+ care at either level of medical education.
The American Academy of Family Physicians has made curriculum recommendations for the care of Lesbian, Gay, Bisexual, Transgender, Queer/Questioning, and Asexual populations. These guidelines state that family physicians should have a "basic understanding of hormonal treatment for transitioning and awareness that these treatments can be provided by family physicians without specialist consult."
This session will explore in depth how some US academic centers are incorporating LGBTQ+ health education and providing safe space and access to quality care for our LGBTQ communities, by providing gender affirming therapy, HIV prevention and treatment, vaccinations, adequate health screening, mental health support and compassionate, non-judgmental management of holistic health care needs.
Educators play an important role in addressing sex, sexuality, and gender in medical education and residency training and thus faculty development is crucial. This session will provide an opportunity to learn about several approaches to developing learning environments that emphasize evidence-based and patient-centered care to LGBTQI+ populations.

Biography

Native of Tegucigalpa, Honduras. Completed medical school at the National University of Honduras and residency training at the IU-Methodist Family Medicine Residency Program in Indianapolis. Associate Professor of Clinical Family Medicine and currently serves as Vice Chair for Education for the Department of Family Medicine. Serves as course director for multiple electives including Domestic Global Health and Healthy lifestyle and Fitness. Beyond his instructional roles, he also serve as mentor for multiple medical students and advisor for Medical Student Interest Group such as Latino Medical Students Association, Medical Spanish interest Group, and the new Lifestyle Medicine Interest Group. Serves as outpatient preceptor at the IU-Methodist Family Medicine Residency creating innovative ways to engage education and wellness in residents , Coordinates the Family Medicine-Methodist Global health Track, LGBTQ health tracks, lifestyle Medicine Curriculum. Active member of US National Organizations such as AAFP, ACLM. Serves as a Consultant for the National University of Honduras in the creation of a family Medicine Residency Program.

Demystifying gender affirming care: The critical role of GPs in providing care for transgender patients

Summary

Gender affirming care lies firmly within the scope of general practice and family medicine.

General Practitioners (GPs) and Family Physicians have the necessary skills and knowledge required to provide high-quality gender affirming care to transgender patients, however, many practitioners feel they do not have the confidence or experience to do this. This creates barriers for patients, and limits their access to care.

This session strengthens knowledge and confidence in the provision of quality medical care and gender affirming treatments for the transgender community. Underpinned by principles of the informed consent model of care, an understanding will be developed of issues and barriers faced by the transgender community and how GPs and Family Physicians can be a professional, safe and vital part of their medical team.

This session builds empathy and understanding of the diverse experiences of the transgender community, building the capability of GPs to be vital advocates and allies, and positioning transgender health care as a human rights issue.

Biography

Dr Portia Predny (she/her) is a General Practitioner who works in Sydney, Australia. She has a special interest in LGBTIQ+ healthcare, and particularly enjoys working in areas of medicine where her clinical interests and passion for social justice collide. She practices as a Gender Affirming clinician under the Informed Consent model of care, and has extensive experience in the care of transgender patients. Portia is a member of the Board of Directors of AusPATH (the Australian Professional Association for Trans Health) and is passionate about reducing health inequality and improving access to care for gender diverse people. As such, she enjoys providing professional development to other clinicians to increase their skills and confidence in this vital area of health care.
