Standards of Care and Clinical Practice Guidelines in the Era of Universal Health Care

Summary

Strengthened by the Universal Health Care (UHC), health care reforms paved the way in achieving the provision of equitable, comprehensive and quality health services from health promotion, disease prevention, treatment, rehabilitation and palliation without causing financial hardships. Appropriate health services should be defined and set through the standards of care and clinical practice guidelines in order to ensure that the health services are of quality and evidence based. In the Philippines, through the enactment of the Republic Act No. 11223 “Universal Health Care Act,” the Philippine’s health department is mandated to develop clinical practice guidelines and set standards of care that are of quality and evidence-based in order to ensure quality health services and improve patient outcomes. In Myanmar, the national health plan was revised in 2016 to include the plan for UHC. General practitioners (GPs) were considered as the main providers of essential healthcare services and Myanmar GP Society published the GP guidelines in 2018 to set up the quality standard for all GPs. This session aims to describe the process and share the experiences, particularly the involvement of young doctors, in developing standards of care for health service delivery in South-East Asia. Young doctors from the Philippines and Myanmar will share their experiences and involvement in the development of standards of care and clinical practice guidelines. This will be followed by a facilitated group discussion exploring the experiences and involvement of the participants in standards setting and clinical practice guidelines development in their own countries.

1. Importance of standards of care and clinical practice guidelines in health service delivery in the era of Universal Health Care;
2. Describe and compare the process of the development of standards of care and clinical practice guidelines in other countries; and
3. Compare the development of standards of care in public and private perspective.

Biography

Mel is a primary care doctor currently working as a medical officer at the Department of Health in the Philippines wherein he is in charge of research evaluation and management and is involved in the development of integrated standards of care for various life stages and in the appraisal of clinical practice guidelines as part of the national practice guidelines program. He previously worked as medical officer at the Health Promotion Bureau wherein he was the national program manager of the tobacco prevention and control and in the program and control of fireworks injury. He was involved in crafting health policies and position papers on NCDs, tobacco, alcohol, transfats, and other social determinants of health. He also lead the Covid-19 vaccine social listening arm of the national task group on demand generation and communications.

Assessment of undernutrition and it's correlates among elderly patients attending General out patient clinic of Federal Medical Central Keffi

Summary

Background: Globally, the number of elderly people is on a steady increase. Epidemiological studies show that 11% of the world’s population is over 60 years of age with a projected increase of 22% by 2050. Undernutrition, a form of malnutrition is a state in which a deficiency of nutrients which cause measurable adverse effects on body composition, function or clinical outcome- multi-systemic effects in the elderly. 
Objective: This study aims to determine the level of undernutrition and its correlates among the elderly in order to form the basis for intervention and treatment on nutritional problems in the elderly. 
Study Design/Setting: A cross sectional study involving 122 elderly study participants attending.
Methods: A total of 122 participants were recruited into this study through systematic sampling technique after obtaining a written informed consent from them. Data collected from the study participants included socio-demographic data, malnutrition indicator, family functioning, depression scores.
Results: The mean age of the study participants was 67.5 ± 7.2. The prevalence of undernutrition was 25.81% with 54.8% of them at risk of undernutrition. There is significant correlation of age (r = - 0.256, p = 0.01), Quantity of  full meal consumed per day (r = 0.31 p =0.01), fluid intake per day (r=0.27 p = 0.03) ,mobility (r = 0.41 p at 0.00), BMI (r = 0.53 p at 0.00), polypharmacy (r = 0.23 p at 0.01), calf circumference (r = 0.20 p at 0.03), presence of pressure ulcers (r = 0.50 p at 0.00), self- view of nutritional state (r = 0.54 p = 0.000) and depression scores(r = -0.26 p = 0.010) with level of undernutrition.
Conclusion: This study found the prevalence of undernutrition and determinants in the elderly to be high which underscores the need for nutritional interventions to help curtail this menace.

Dr Sonny John Kumbet is a consultant Family Physician and Geriatrician. He works with Federal Medical Central Keffi, Nasarawa State, Nigeria. He is the current head of unit of the Geriatric Medicine under Family Medicine department. He is passionate about Family Medicine with more passionate about elder care. He is the current group manager( secretary ) of Afriwon Research Group. We have some publications and currently working on 3 papers. He is also the President of Holistic Global Medical Mission, a Non Governmental organization whose sole aim is to meet the high health care needs of the less priviledge people in Nigeria. He is highly interested in research and rural medicine were he practice Family Medicine and Geriatric Medicine.
