Summary

Keeping kidneys integrated care pathway is an innovative tripartite platform of GP facing functions, GPwSI service delivery and nephrologist facing functions. Clinical services are delivered in a community health centre in a Queensland township, that has no specialist nephrology services.

Biography

Dr Gary Chang works as a GP with Special Interest in Chronic Kidney Disease within the Metro North Kidney Health Service, Brisbane. He also works as a General Practitioner in Moorooka, an culturally diverse inner Brisbane suburb. He has special interest in exploring innovative models of care for chronic diseases management to improve patient satisfaction and outcomes. He is also currently involved in a joint community viral hepatitis clinic with the Mater Hospital, Brisbane.

Dr Trina Gregory is an energetic, experienced GP with a passion for developing systems efficiency compliant with current standards and safety into patient care. Dr Gregory has been using point-of-care testing in general practice for the last 30 years.

Dr Nick Hartland is the First Assistant Secretary of the In Home Aged Care Division in the Department of Health. Nick’s key focus is supporting greater independence and wellness for Older Australians, including entry-level aged care assistance at home (Commonwealth Home Support Programme) and assistance for older Australians with more complex care needs (Home Care Packages Program). Previous roles Nick has held include: Aged Care Policy Principle Advisor, Chief of Staff to the Hon Ken Wyatt, AM, MP, Minister for Seniors and Aged Care and Minister for Indigenous Affairs, other Executive roles within the Department of Health. Nick has a PhD in Criminology from Melbourne University and a Bachelor Degree in Political Science.
