Details
This session introduces practice owners to a streamlined approach to patient identification activities (clinical audits) that integrates expert external support to drive business efficiency, clinical excellence and Quality Improvement (QI) opportunities.
The presentation will explore how this model addresses the core challenges of modern general practice: •For Practice Owners & Managers: Discover how supported audits identify "missing" patient cohorts, ensuring appropriate billing, optimising Medicare revenue, and fulfilling QI requirements for accreditation whilst embedding sustainable processes into everyday clinical care. •For Doctors: Understand the transition from manual data searching to a nurse-led, taught and integrated workflow. This allows GPs to focus on high-value clinical decision-making and patient care rather than data mining, while easily meeting RACGP CPD requirements for "Measuring Outcomes." •For Practice Staff: Learn how the involvement of Wellness Clinical Services Nurse Educators reduces the technical burden on practice nurses and reception staff, providing a turnkey patient identification framework •For Patients: See the direct impact of proactive identification, ensuring those with chronic or progressive conditions (such as Multiple Sclerosis) are recalled for timely reviews and commenced on GPCCMP, access to high-efficacy therapies, and improved long-term health outcomes.

Driving growth and better care: The power of supported clinical audits
11:20 - 12:05Biography
Holly is the Program Lead at AXE Health Services, specialising in the design and delivery of nurse led disease education programs built for real general practice environments. With over 10 years on the ground as a Practice Manager in regional WA and Melbourne, she brings a firsthand understanding of the competing demands placed on practice owners and their teams - and the daily pressure of keeping a practice running efficiently while still delivering quality care.
That experience has shaped her belief that the best processes are ones that are simple to implement and integrate into existing workflows rather than disrupting them, for lasting habitual changes.
Holly is passionate about the role that programs and clinical education play in creating structured opportunities to identify and recall patients - supporting both patient outcomes and the sustainability of the practice with increased reviews and billing opportunities.
