The Brisbane Fellowship and Awards Ceremony is a premier event on the Royal Australian College of General Practitioners (RACGP) Queensland calendar. It's an opportunity for the general practice community to honour New Fellows, Rural Generalists and Fellows in Advanced Rural General Practice to celebrate their achievement of Fellowship.

Providing your next of kin details is optional but recommended in case of an emergency during an event.

As an RACGP member you must provide your six digit member number to confirm your Fellowship has been ratified.

You cannot complete this registration form without your RACGP member number. 

To assist the Censor with pronouncing your name correctly on stage, please provide the phonetic spelling using the below format:

• Break your name into syllables and separate with dashes.
• Capitalise syllables that are emphasised 
• Include all letter sounds that might be helpful, even if those same sounds are not present in the spelling of your name.

Please be aware that whilst every effort is made to ensure menu items are allergen free, in the case of specific allergies or medical dietary conditions it is recommended that you consume food and beverages at your own discretion and carry an Epi-pen if recommended by your health care providers.

Menu items may still contain traces of allergen ingredients if they are prepared in venues and/or catering facilities that use and process these products as ingredients in other menu items.

The RACGP will provide you with an academic gown to use during the ceremony.

RACGP Aboriginal and Torres Strait Islander Health provides a gift of one ceremonial sash to Aboriginal and/or Torres Strait Islander Fellows to wear with the ceremonial gown.

Due to venue capacity, you may bring a maximum of four (4) guests.

Please advise below if you or your guest/s require special requirements (e.g accessibility requirements, prayer room, parents' room etc). 

In accordance with regulations and requirements which came in force on 25 May 2018, the following consent is required before you can proceed and register for the conference. Note: if you do not give consent for your details to be used by the organisers, we will not be able to contact you regarding the planning and conduct of the conference, you will not be able to register for the conference and you will be re-directed back to the main website.
